Yes, there is a form to fill out. However, it’s quite short and it only takes about 5 minutes to complete. Most people prefer to download the form and complete it ahead of time, but you can also fill it out in the waiting room, if you prefer.

If you plan to use your insurance to pay for therapy, call the number on the back of your card to verify your benefits. If your insurance is one that I participate with, bring your card with you to your first visit. (see Fees for more info.)
